Well it seems that MC and his Girlfriend both died , and got to God.
MC was unconscious , so his girlfriend asked for them to be reborn as diffrent genders?
Also she somehow got the hands on typical mind control app and used it on MC.
They seem to get together again in the end , albeit in the reversed positions :D

Sort of, but you have some details wrong.

MC (27 years old) works for an Eroge maker. The girl (20) is his kouhai, a new employee in her first year, but not a girlfriend. (She's unusual in the sense that she's a girl who enjoys guy-aimed eroge.) They are in crunch time, getting close to when their last game, an eroge that centers on a mind control app, is supposed to go gold (22 days according to the calendar in the background). The kouhai girl is all pumped to stay overnight pulling an all-nighter (and has prepared a cardbox box for doing so), but the guy says that their company doesn't do that (and not to believe that the anime "New Game!" is realistic.)

They walk together to the train station to leave, get hit by truck (he dies saving her.) He recognizes the world as being their game world (and makes a 転スラ / "That Time I Got Reincarnated as a Slime" comment about how he's going to get amazing cheat skills and get a harem). After various scenes inside the world of that game, he meets the kouhai, who is the main male character. S/he reveals that when she died, she was the one visited by the goddess. Apparently her one regret in life was not playing that game to see how it turned out, considering all the hard work they spent debugging it. There's an amusing scene where the goddess tries to suggest that it would be better for her as a girl to maybe be reborn into an otome game, but the girl insists, and what's more, wants to be the main character because her secret dream is to see what it's like to have a dick and fuck cute girls.

The MC apparently just got sort of caught up in the whole scenario by mistake, according to the girl and the goddess. The goddess didn't realize that there was another spirit and ended up reincarnating the guy as well into the main heroine. Oops.

The girl turned MC uses the hypnotism app on the guy turned main heroine in order to get help capturing all the other girls, since the senpai has more experience at eroge (including working on them) and thus can figure out all their flags. After they achieve the True End (which is a graduation event involving the two of them, since senpai is now the main heroine after all), the goddess appears and as a special service for achieving the True End, gives them the chance to reincarnate back in their original world, at the cost of their memories in the game world. Their choice is...

to remain in the game world and not be reincarnated. Instead, they continue the lives of the eroge characters after graduation. This game world *also* has eroge companies, and they end up working at an eroge company again. The guy turned girl still acts as senpai and knows more about programming, but the girl turned guy can still use the hypnotism acts to put commands on MC like "at 11 PM you will become uncontrollably aroused" while MC is trying to debug the program. (This apparently happens often, and is referred to as "Cinderella Time" but as being a little (an hour) early this time, a reference to the somewhat different effects at midnight in Disney's Cinderella.) The kouhai was possibly just meaning to tease the senpai, but senpai takes the phone with the app and uses it to make the kouhai be unable to ejaculate until he fully satisfies her needs. Happy End, inside the game world.

@Thacker You also got some of the details wrong. The MC made his comment about cheats and harems before he realises that he was reincarnated into the world of the eroge. He knew he wouldn't be getting any cheat skills once he realised that he had reincarnated as the heroine of an eroge.

The reason the kouhai was reincarnated into the eroge world is because she was supposed to be reincarnated into the world of the game she played the most and she played the eroge a ton of times for the sake of debugging. There is no mention whatsoever of her only regret in life not being able to play the game.

Also, when the goddess gives them the chance to reincarnate into their original world, she says that they would lose all their memories which implies that they would lose the memories of their past live and not just their memories of the game world.

Yes, thanks, as you state, the goddess says it's the game she played the most in her previous life that's she's going to into due to karma. But she still is very interested in going into that game and insists upon it (saying that it's her dream, as I said) when the goddess suggests switching to a otome game instead. And at the true end part the goddess says lose all their memories without specifying; I agree that the implication is likely as you say, but it's implied and could be read the other way.

I am amused by some of the writing, including the male MC at the beginning thinking "Oh, I get it, this is like one of those reincarnated as the villain in an otome game things that's going around, only in an eroge." Poor guy, thinking that he's the protagonist when just swept along.
